Eligibility statement

Important: This ballot belongs to the person listed on the voter information letter and should not be used by anyone else.

I, being a voter entitled to vote in this election, declare that:

I am at least 18 years old (or will be by final voting day);
I am a Canadian citizen;
I am a resident, landowner or tenant of the applicable Quinte West ward (or the spouse of one);
I am the person listed on the voters’ list and/or election document; and,
I have not already voted in this election.

I affirm that this ballot was assigned to me and that I am casting it independently and of my own choice.

I make this statement conscientiously believing it to be true, knowing that it is of the same force and effect as if made under oath in a court of law, and with the understanding that it is illegal under Ontario’s Municipal Elections Act to:

Vote when you are not eligible;
Vote with more than one ballot;
Give or sell your voter information letter, voting PIN or ballot to someone else;
Take or buy someone else’s voter information letter, voting PIN or ballot;
Force or bribe someone to vote in any way;
Accept a bribe to vote in any way;
Invade the privacy of someone who is voting;
Intentionally reveal how someone else voted;
Stop someone from voting;
Lie to or intentionally mislead and election official; or,
Try to do any of the above.

I understand that, if it is discovered that I have committed any of the above offences, I may be charged under Ontario’s Municipal Elections Act and subject to penalties including a fine of up to $25,000 and a prison term of up to 6 months.
